Team Check in Will Be
Friday, April 15, 2011 from 6-9PM.
The check in will be at the Embassy Suites, 9940 Coprorate Campus Drive, Louisville, KY 40223.
The Following Credentials Required at Check-In
USYSA affiliates
• USYSA player & coaches cards; must be laminated; must have photo & DOB
• USYSA official team roster approved by state association
• State travel permit
• Guest-player form (max. 3 guest players per team)
• Medical release form (signed and notarized) for each player on roster; these can be from your own State Association
• Javanon Cup liability waiver; signed by all players & parents
US Club Affiliates
• Player & Coaches cards; must be laminated; must have photo & DOB
• Association or Organization approved roster
• Letter of Good Standing from your Association or Organization
• Guest-player form (max. 3 guest players per team)
• Medical release form (signed and notarized) for each player on roster; these can be from your own State Association or Organization
• Javanon Cup liability waiver; signed by all players & parents
